Adult MH Case Management |
Case Management Services provides assistance to individuals with severe
mental disabilities to obtain the services they need to live productively in
the community. Because services can be fragmented in many areas,
case managers provide a critical function to monitor a person's needs and
assure that appropriate agencies get involved.
Services may include:

Assessment of individuals consumer's
needs.

Assist in the development of an
individualized service plan with the consumer and family, (if involved
in the consumer's treatment).

Assist in linking the consumer to
services and community support systems.
-
Meet with the consumer in community
settings, assist with making and keeping appointments, support community
living and keep consumer engaged with services.
-
Advocate on behalf of the consumer and
consumer’s family.

Coordinate discharge planning from state
and local hospitals and residential facilities.

Adult MR Case Management |
HNNCSB Case Management Services are committed to providing optimal community
services for individuals with Intellectual Disabilities by assessing
consumer strengths and needs, linking consumers with community services and
supports, monitoring consumers response to treatment and the quality of care
provided, facilitating Person Centered Planning to fulfill a consumer choice
of employment, living options, and socialization, ensuring effective crisis
stabilization services if needed

Person Centered and carefully crafted to individual needs, cultural values
and preferences.

Adult SA Case Management - Female |
Project LINK is an interagency, community based program aimed at assisting
women and their children who have been affected by substance abuse. Through
the use of innovative linkages, a full continuum of care is provided that
integrates prevention, early intervention, and substance abuse treatment
with health care and other support services.
The goal of Project LINK is to
support a woman's journey to a healthy lifestyle for herself, and for her
family, whenever possible.
Our mission is to ensure that
all women whose lives are affected by substance abuse are linked with
resources that foster and support their physical, emotional, spiritual, and
family well-being.

Adult Male Mental Health and Dual Diagnosis Case Management |
Case Management provides
assistance to individuals with severe mental disabilities to obtain the
services they need to live productively in the community. Because services
can be fragmented in many areas, case managers provide a critical function
to monitor a person's needs and assure that appropriate agencies get
involved.
Case Managers can be nurses,
social workers, or mental health therapists. The Case Manager will
work with you and/or your loved one to develop an individualized plan that
will build on your strengths and preferences to have control of your
disability.

C&A Case Management |
Case Management is a service
offered by Hampton-Newport News Community Services Board to help families
with children who have behavioral health needs. Case Management assists
children and families in gaining access to needed medical, social,
educational, and other services. In addition, Case Management ensures that
services are coordinated among all of the agencies and care providers.
Upon starting Case Management
Services, a case manager will meet with the child and family to develop a
care plan with goals and identification of services available in the
community. Services may include:

COS Adult Outpatient Services
|
Comprehensive Outpatient Services is
a dynamic clinical practice offering a full spectrum of outpatient
mental health services. We treat adults eighteen years of age
and older, including the geriatric population. Our
comprehensive approach begins with a full clinical assessment of a
client's history, current symptoms, and goals. Individual,
group, family, and couple psychotherapy and psychiatric services are
tailored to meet an individual client's specific needs.
Services are provided in a clean, attractive environment by a
courteous, professional, well trained staff. As part of
a large, well integrated, full service agency we are able to offer a
holistic treatment approach to maximize an individual's mental
health and wellbeing.

COS Child and Adolescent Services |
We provide Medication
evaluation and monitoring, Outpatient psychiatric treatment,
Psychotherapy ,Individual &
Family, Assessment,
Psychological testing, including
intellectual and emotional functioning, Court-ordered (forensic)
evaluations, Psychological evaluations for the purposes of differentiating
diagnoses.
All treatment providers are licensed professionals
with extensive experience in working with children and families.

Creative Options |
Creative Options is a Medicaid Waiver Day Support program for
individuals who are intellectually disabled. The program provides
services in an environment where individuals have the opportunity to
engage in activities that enhance self development and promote community
involvement. |
Emergency Services |
Emergency Services is a team of
counselors that perform mental status assessments and crisis intervention
services which may include counseling, making referrals to appropriate
resources or coordinating admissions to treatment services. The goal of the
mental status assessment is to determine if mental health services are
needed and to exhaust all least restrictive interventions available prior to
making recommendations for inpatient treatment.

Roads Clinic

- The
Opioid Treatment Program at
the Hampton Roads Clinic offers a unique and specialized program for
adults with a long history of dependence on opiates such as prescription
medications, heroin, and morphine. The program offers a full range of
services to help individuals achieve recovery from opiate dependency and
improve the quality of life. Our services are provided by licensed or
certified clinicians who specialize treatment of substance abuse.

IDD Services Administration |
The
Intellectual Developmental Disability Services (IDDS) Division
provides services to individuals 18 years of age and above, who have
an intellectual disability and reside within the Hampton and Newport
News area. Our Division operates three programs for adults with
intellectual disabilities: Creative Options Adult Day
Services, Residential Services, and Adult Case Management Services.

Lassen House Vocational |
*Lassen House is a
psychosocial clubhouse which offers each member a sense of importance,
ownership, and empowerment by encouraging his or her contributions in
keeping the clubhouse alive and operating. This objective may be achieve
through member participation, involvement, and task completion in one or
more of the following units: Horticulture, Environmental, Thrift Store,
Clerical, Food, Snack Bar, Kitchen, and Outreach. These units also provide
opportunities to learn daily living skills and/or prepare for work tasks
and employment in the community. An onsite Job Coach is available to work
with members attending Lassen House that are interested in obtaining
vocational training and employment. In addition, Lassen House provides its
members with a variety of therapeutic and recreational groups and
activities.
Hours of Operation:
Lassen House is open Mondays
through Fridays from 7:30 A.M. - 2:00 P.M. Holiday, Evening, and
Weekend activities are held on a periodic basis.
*Vocational Services Program
is a Supportive Employment program. Which assists consumers of the CSB in
obtaining and maintaining integrated employment. We work directly with
the Department of Rehabilitative Services to meet all vocational
oriented goals of our consumers. We are different from traditional ESOs
because as a part of the CSB we are able to provide training in various
job related areas. For example, we have groups that aid in the
understanding of how working effects SSA benefits and groups that
discuss how to correctly fill out applications.

Mentor Services and Mental Health Support Services |
Therapeutic
Mentor Services
Mentor services provides in and out of
home education, behavioral interventions, training and crisis management
services to children and adolescents with moderate to severe emotional
disturbances, mental retardation, or substance abuse and their families.
Includes the provision of crisis intervention, behavioral interventions,
participation in treatment planning, outreach service provider linkage,
monitoring, advocacy, daily living support, respite, independent living
skills, vocational instruction, medication monitoring, supportive counseling
and positive role modeling. This service is funded by Hampton and Newport
News CSA. Medicaid funding is not applicable to mentor services. Youth
must be approved for funding through the Family and Assessment Planning Team
(FAPT) process.
Mental Health Support Services (MHSS)
Mental health support services
are training and support to enable individuals with significant functional
limitations to achieve and maintain community stability and independence in
the most appropriate least restrictive environment. This service focuses on
obtaining independent living skills and is therefore appropriate for older
adolescents.

provided by Qualified Mental Health and Mental Retardation Professionals
– Supports
community stabilization for individuals with significant functional
limitations

Performs behavioral interventions
to stabilize individuals in the least restrictive setting
–
Provides daily living support,
independent living skills training, monitors health, nutrition and physical
condition, monitors medication compliance and assists client with accessing
community resources
– Funded
by Medicaid
Mental Health Support Staff will
provide training in or reinforcement of functional skills and appropriate
behavior related to the individuals health and safety, activities of daily
living, use of community resources, assistance with medication management,
and monitoring health, nutrition, and physical condition.
To be eligible for this service
the individual must demonstrate a clinical need arising from a condition due
to mental, behavioral, and emotional illness that result in significant
functional impairments in major life activities. Individuals must meet
certain criteria on a continuing or intermittent basis:
- Have
difficulty in establishing or maintaining normal interpersonal relationships
to such a degree that they are at risk of hospitalization, homelessness, or
isolation from social supports.
- Exhibit
such inappropriate behavior that repeated interventions by the mental health
social services or judicial systems are necessary.

Exhibit difficulty in cognitive
ability such that they are unable to recognize personal danger or recognize
significantly inappropriate social behavior.
- Require
help in basic living skills, such as maintaining personal hygiene, preparing
food and maintaining adequate nutrition, or managing finances to such a
degree that health or safety is jeopardy.

Partners in Recovery/Next Step Day Program |
Partners in Recovery:
Our
Co-Occurring Disorders program provides short term treatment for adults who
require
both comprehensive
psychiatric/addiction evaluation and stabilization of their psychiatric
symptoms and substance abuse. The program is uniquely designed to offer a
comprehensive approach, integrating mental health and addition treatment in
a single program design.
A
team model of treatment emphasizes assessment,
brief treatments and crisis intervention, clinical case management and the
initiation of rehabilitation. Members of the treatment team integrate
psychotherapy, medication management (if indicated), relapse prevention and
life skills training with 12-step recovery, crisis intervention and social
developmental orientations to support participants in achieving their goals.
Next Step Day Treatment:

The
Next Step
Day Treatment Program
integrates the treatment of substance use disorders with mental
health therapies to form the Next Step Day Treatment Program.
Combining the resources of both fields results in a dynamic program
capable of addressing the needs of consumers having one or several
diagnoses, such as those individuals with alcohol or drug dependence
or simple depression and/or anxiety, as well as those individuals
with addiction or multiple substances also suffering from bi-polar
disorder, depression, PTSD, and/or compulsive behaviors. The
consumers we serve regularly have some type of mental disorder
co-existing with an addiction. The disorders may be independent of
each other, yet they invariably influence each other to some
degree. |

Pathways
Independent Living Program |
The Pathways
Program, operated by the Hampton Newport News Community Service Board in
partnership with the Hampton and Newport News Departments of Social
Services, is a moderate supervision independent living program
The program serves male and female young adults
between the ages of 16-21 years. Clients are assisted in acquiring,
practicing, and implementing the daily living skills that will allow them to
successfully transition into their communities as contributing, self
sufficient adults. Pathways provides a full array of therapeutic and
independent skills training in a structured homelike setting with 24 hour
supervision. |

Prevention Services
|
Our programs include:
Strengthening
Families Program - Facilitators lead 14 once a week sessions filled with
fun, informative activities, and discussions designed to promote healthy
family interactions. Included are Parenting Skills, Children's Skills
and Family Skills.
The LifeSkills Training
Program - This is a proven, highly effective substance-abuse prevention
and competency enhancement program designed to focus primarily on the major
social and psychological factors promoting substance use/abuse.
Community Builders Network
of Hampton Newport News -
The Community Builders' Network,
known as CBN,
is a 501c3 non-profit
organization, comprised of community based coalitions, which exist for the
purpose of conducting and supporting activities in the development and
caring of youth and adults. The numerous coalitions which make up this
organization collectively offer alternative actives to youth and adults.
These activities focus on eliminating alcohol and substance abuse,
strengthening family structure and other positive activities for youth and
adults in the neighborhoods of Hampton and Newport News.
CBN
collaborates with all member coalitions to
provide technical assistance, planning, education and training as a means
for facilitating coalition based activities. With an underlying focus of
stimulating youth to adult partnerships, CBN
values all input from youth members in all
aspects of program operations.
The CBN
network currently has over 100 community coalitions from
across Hampton and Newport News.
