Case Management provides assistance to individuals with severe mental disabilities to obtain the services they need to live productively in the community. Because services can be fragmented in many areas, case managers provide a critical function to monitor a person's needs and assure that appropriate agencies get involved.
Case Managers can be nurses, social workers, or mental health therapists. The Case Manager will work with you and/or your loved one to develop an individualized plan that will build on your strengths and preferences to have control of your disability.
